What is a HMR?

A HMR is a Home Medicines Review.
A Home Medicine Review involves your Doctor and a specialist pharmacist (known as an Accredited Pharmacist) working together to ensure maximum health benefits from your medications.
Your Doctor organises the review by sending a referral to an Accredited Pharmacist. The Accredited Pharmacist is a specialist in many areas of medication and is there to review and provide you with all the answers you may need.
The Accredited Pharmacist contacts you directly and organises a time to come to your home to conduct the interview. After the interview a report goes back to your Doctor, who will discuss with you any recommendations and may make appropriate changes to your medication regimen.

How much does it cost?

HMRs are free to the patient and are bulk billed to Medicare by the clinic.

What is an Accredited Pharmacist?

An Accredited Pharmacist is a specialist pharmacist with a qualification that allows them to perform home medicine reviews. They are there to address your concerns, answer your questions and provide education regarding your medicines.

Can I order a HMR for someone else?

If you would like to have a HMR done for you or a family member directly without Medicare please contacts us to engage an Accredited Pharmacist directly.
The Medicare review process begins with a GP sending a referral to an accredited pharmacist. There is currently no way to get a HMR done through Medicare without this process. You can download the patient information sheet here .

Do I need to have a HMR done regularly?

You should be asking your GP about a HMR if you are concerned about your medication or have recently been in hospital.

Can I have this out of my home? / I don’t want someone to come to my home can I go to them?

The benefits of a HMR is that an accredited pharmacist can provide you with education and feedback on all your prescribed medicines and over-the-counter medicines that you have in your home.
Your individual circumstances are important and as such please communicate your concerns with your doctor.

Can my carer be present?

Yes, it is highly advisable to include your carer as they will have a good knowledge of your circumstances.

English is not my first language – what can I do?

Raise this concern with your referring Doctor. In some circumstances it may be necessary to have a translator present or request a specific Accredited Pharmacist with that may speak your language.

Is my personal information secure?

Only your Doctor and your Accredited Pharmacist have your information and everything is sent through Australia Standards compliant encrypted communications. Your information is safe.

I don’t have access to Medicare, Can I still have a home medicine review?

Yes, If you would like to have a HMR done for you or a family member directly that doesn’t have access to Medicare please contacts us to engage an Accredited Pharmacist directly.
